I have a monthly plan with 1-week trial introductory offer for iOS. I use RevenueCat Paywall. If I add a second introductory offer with a discount for the first 3 month, would it work?
The other solution is to create a new subscription with discount introductory offer, but without free trial and a new paywall?

Unfortunately, I don’t believe you can have two introductory offers for the same product in iOS. When you try to add the second one in App Store Connect, it’ll replace the initial one.
So I’d go with your second option - add a new sub with the your new intro offer and users can choose between on your paywall. The caveat here of course is that users could use up their free trial on one product, and still qualify for the discount on your second product, unless both products are in the same subscription group.

I see now, it’s duplicated, when I duplicate the offer.
