Repeated purchases without actually being charged

I'm experiencing an issue where some users are able to make repeated purchases without actually being charged. The problem is that multiple purchases are being reported with the same transaction_id as the original purchase. From the user's perspective, it looks like a new purchase was made, but no new transaction or payment actually occurred in the App Store.

This leads to users getting access multiple times without paying again.
