I'm being told there should be a way to make a one-time payment to stat the membership. Then every five years there can be a required maintenance fee to continue the membership, is that doable? And would the play store and app store accept this in an app published there?

You can create a non subscription purchase and then you would have to manually check every 5 years to make them pay a new fee. You can check the documentation here.
